Medication Administration in School
If your child must have medication of any type during school hours, INCLUDING over-the-counter medications, you must adhere to the following regulations, under state law:
All medications must have the order to administer at school. The form should be completed by the doctor and parent.
Please use one of these forms:
- All medications must be brought to school in the original container and delivered by the parent.
- All prescription medications must have a pharmacy label with the student’s name, date of birth, and order information (name of medication, dosage).
- No medication can be accepted by school staff without a doctor’s order (MED-1 form) and written parent/guardian permission. (Doctor’s order must state “to be given in school”).
- All medications must be picked up by the parent when it expires, is no longer needed, or at the end of the school year.
- Some students may need to self-medicate for certain chronic health conditions. Students who need to self-medicate with a prescription drug must have a Med-1 form and the school nurse will review it with the student to authorize self-administration.
- A NEW Med-1 form is needed every school year.
The following medications are available to be administered by me, the school nurse if the student has a current and complete S-865 Form on file. (Refer to Quick Links)
- Acetaminophen (Tylenol)
- Ibuprofen (Advil, Motrin)
- Hydrocortisone Cream 1% (Anti-itch cream)
